You will be responsible for coordinating and providing quality, multi-disciplinary skilled nursing services in the home setting. As part of the interdisciplinary team, our registered nurses are responsible for carrying out individualized patient assessments, developing, implementing and coordinating goal-oriented plan of care with the patient, family and health care provider.
Other responsibilities of the registered nurse (RN) include ongoing evaluations of patient’s response to care and plan modification based on the patients progress. Must be able to be self-directed, possess initiative and be able to work collaboratively with colleagues within and outside of the agency. Experience with electronic medical record documentation is required.
